Instructions to Complete the Transformational Church Assessment Tool (Survey)

Thank you for taking time to complete the Transformational Church Assessment tool (survey).  Your candid input will enable Boones Creek Baptist Church to determine what we have to celebrate and what challenges we face on our journey to becoming a  Transformational Church.

A Transformational Church is defined as: a congregation that is joining God on his mission, sharing the gospel of Jesus Christ and making disciples.  These disciples become more like Jesus, and begin transforming their community and the world for the Kingdom of God.

To complete the survey:

Contact the church office between 9 a.m. to 4 p.m. Monday, Wednesday or Friday and ask Reta for the church’s passcode.  Then,

1. Go to the website http://tc.lifeway.com/ (this is a secure website) and enter the church’s passcode in the green circle marked “First Time User”.

2. Fill in all information to register and create a username and password. This will allow you to login again if you need to stop and finish the survey later.

3. Take survey all questions must be answered.   Your input will not be shared with anyone.  The results are reported by Lifeway without names.
